1. Your child's most recent immunization records

2. Hearing and Vision screening (if your child is 4 or older by September 1st)

3. Food Allergy Emergency Plan (If your child has a life-threatening food allergy)

4. Food Allergy Preference Form (Optional)

5. Health Care Professional's Statement (To be signed by your child's doctor)
