The following documents MUST be on file with the school by June 1. Please submit to documents@bearvalleyacademy.com:

• Completed Admission Form 

• Most current copy of the child's immunization records

• Signed copy of the Health Care Professional's Statement

• Signed copy of child's hearing/vision screening (if the child is 4 or older before June 1, 2020)

• Food Allergy & Anaphylaxis Emergency Care Plan (if needed)

• Authorization for Dispensing Medication Form (if needed)
